LASA (Look Alike Sound Alike) are on the most common causes of medication errors. This is because It doesn’t matter in what order the letters in a word are. This is because the human mind does not read every letter by itself, but the word as a whole. Community Pharmacy has key role in patient safety and there are a number of things which you can do to increase awareness of LASAs.
