Following on from our success in Lewisham and Greenwich NHS Trust, the Health Innovation Network’s aim this year is to support the implementation of a referral pathway between Dartford and Gravesham NHS Trust and Community Pharmacists.

Inpatients who are considered to benefit from on-going support and continuity of care after leaving hospital are to be identified by hospital pharmacists and referred to local community pharmacists. PharmOutcomes will be employed to facilitate the transfer of information between hospital and the patient’s nominated community pharmacy teams to improve the coordination of care. This will include electronic transfer of discharge letters as part of the referral process. Community pharmacists may then provide a follow-up consultation tailored to the individual patient needs.

We believe community pharmacists can support patients to ensure that they get the best from their medicines by providing services such as the new medicines service (NMS) and post-discharge medicines use review (MURs), where changes to medication could be explained, patients could be assessed for adherence support and patient medication records (PMRs) updated with any changes to medicines. As part of this pathway development engagement of both hospital teams (primarily hospital pharmacists and pharmacy technicians but also including the wider multidisciplinary team involved in discharge planning) and community pharmacists is key. We therefore plan to support development of the pathway by inviting you to our launch event on Thursday 27th June 2019.
