Over the past decade, the average number of items prescribed for each person per year in England has increased significantly. For many patients, it might be entirely appropriate to be taking multiple medicines. However, problematic polypharmacy can occur when multiple items are prescribed inappropriately to an individual. As a pharmacy professional you are in an ideal position to work with patients and other healthcare professionals to identify, prevent and resolve medicine-related problems. By optimising medicines in polypharmacy, pharmacy teams can help to ensure that patients receive treatment that is appropriate, safe and effective.
