PCN leads and contractor webinar recording

What is required from you…
Complete form C (Title of Form B- Action plan): Completion is by 11.55pm on Monday 29th January 2024. Please click here for Form C.
Payment will appear on your monthly statement from the NHSBSA as Local Scheme 8 (LES 8).
Desired outcome of this project…
- Increase the number of blood pressure check referrals from GP practice into community pharmacy.
- Increase the number of ABPM checks in community pharmacy.
– note national incentive fee is £400 for 2023/24 (need to provide 20 ABPMs between 1st April 2023 and 31st March 2024 to achieve the incentive fee).
- Improve hypertension treatment to target for our area.
These are some of the key outcomes that will be measured at end of this project.

Brief principles of this project (Reminder)
- Project agreed to run up until end of April 2024
- Funding for each pharmacy hosting PCN lead= £1,600 total (based on receipt of 4 online forms at £400 each). This is £67,200 of funding available for our pharmacies through this project.
- Focus of project is to increase uptake and roll-out of hypertension case finding, focusing on ad hoc checks referred in by GPs.
- Key part of the ask is to provide leadership and support to local pharmacies in PCN and engage with PCN team (ICB team are working to make PCN teams aware of this project to facilitate engagement).
- Important chance to demonstrate the importance of these roles and impact that they can have locally on building relationships with PCN teams and driving services.
- The set-up (i.e., forms) is to reduce the admin burden on PCN leads and to maximise time available to do the work locally. They will capture only essential information to evaluate the project and demonstrate value.
